After you find out what immunizations you will need, please call a Travel Clinic to receive the vaccinations. Insurances do not usually cover travel vaccines, but check with your individual insurance company to be sure. Also, try comparing prices for the vaccinations at different clinics, Costco and Walgreen's. Here is a clinic we refer to often:

An Advance Directive are instructions given by individuals specifying what actions should be taken for their health in the event that they are no longer able to make decisions due to illness or incapacity, and appoints a person to make such decisions on their behalf.The link below will connect you to a page where you can print off an Advance Directive packet. If you need assistance filling it out, please schedule with your provider to review it with you. We do request a copy of the packet after you are finished to keep in your chart.
